BLOOM Nurse – Perinatal Navigator

About infants in BLOOM
infants in BLOOM is looking for a compassionate, organized, self-directed Registered Nurse to serve as Perinatal Navigator for Switzerland County. In this role, you'll manage the full Healthy Start case load for the county while also directly supporting Switzerland County families enrolled in BLOOM — combining hands-on nursing care with case management to connect families to the clinical care and community resources they need before, during, and after pregnancy.
What You’ll Do
Provide case management and care coordination for Switzerland County families using a family-centered, strength-based approach.
Conduct outreach and recruitment for both BLOOM and Healthy Start, co-enrolling eligible families in both.
Share BLOOM resources anywhere you're recruiting for Healthy Start, including at surrounding county medical offices, WIC, hospitals and tertiary care centers where Switzerland County residents deliver and receive care.
Facilitate group-based health and parenting education sessions.
Build and use a referral network connecting families to clinical care and community resources.
Support healthy, safe infant and toddler care practices through prevention-focused interventions.
Represent the program with local, state, and national partners, including Fetal Infant Mortality Review (FIMR) and Maternal Mortality and Morbidity Review Committee (MMMRC) committees.
Keep participant records current in REDCap®, with real-time or near real-time documentation.
Provide BLOOM families with immunizations and other interventions per their BLOOM care plan and use BLOOM's digital tools to stay connected with Healthy Start families too.
What We’re Looking For
Active, unencumbered RN license in Indiana
Current BLS/CPR certification
Experience in maternal-child health, perinatal care, or case management preferred
Comfortable facilitating group sessions and building relationships with participants and partner agencies
Organized, self-directed, and comfortable with documentation deadlines and data systems like REDCap®
A warm, strength-based approach to working with diverse families and communities
Reliable transportation and willingness to travel to surrounding counties for outreach and hospital visits
