infants in BLOOM is a mission-driven, woman-owned, and woman-led pediatric in-place care organization serving vulnerable infants and caregivers in rural southeast Indiana. We provide comprehensive, in-home pediatric care from birth through 36 months, meeting families where they are - in their homes, in their communities, and at their most critical developmental window. And if an infant’s placement changes, our care moves with them to ensure absolute clinical continuity. BLOOM serves as the primary pediatric provider for our families, including administration of all required vaccinations as part of a state pilot. We work in close partnership with local health departments and the Department of Child Services (DCS). 

Overview

We are seeking a Nurse Practitioner to join our growing clinical team with considerable experience in newborn and early childhood care. You will serve patients across a seven-county region in southeast Indiana, conducting in-home visits and telehealth appointments on a schedule that works for you. 

Key Responsibilities: 

  • Provide primary pediatric care for infants and young children from birth through 36 months through in-home clinical visits, while following a novel care plan. Telehealth visits as needed.  

  • Assessment and support of caregiver wellbeing through coaching and education. 

  • Collaborate with BLOOM team, DCS, CASA, local health departments and other community resources. 

  • Document clinical encounters using our modern, AI-assisted technology platform. 

  • Participate in care coordination, trainings, team meetings, and call rotation. 

Preferred Qualifications

  • Licensure: Active, unrestricted Indiana NP or PA licensure (or immediate eligibility). PNP preferred, FNP or NNP. 

  • Certifications: Board Certified or Board Eligible in one of the following specialties:

    • Pediatric Nurse Practitioner (PNP) — Highly Preferred

    • Family Nurse Practitioner (FNP) with strong early childhood experience

    • Neonatal Nurse Practitioner (NNP) with early childhood experience

  • Logistics: Valid driver’s license, reliable transportation, and willingness to travel within the 7-county southeast Indiana region.

  • Experience or genuine interest in vulnerable newborns through 36 months. 

  • Comfort with home visits, rural health, and community-based care. 

  • Strong communication skills and alignment with BLOOM's mission-driven culture.
