Essential Duties & Responsiblities

ABC Parenting Program co-delivery 

• Complete parent coach training and certification in the Attachment and Biobehavioral Catch-up model. 

• Deliver the ten-to-twelve-session in-home ABC curriculum to BLOOM families. 

• Complete session documentation and participate in fidelity review and coaching feedback. 

In-home practical support 

• Reinforce safe sleep environments and assist with child-proofing the home. 

• Reinforce caregiving strategies introduced by the clinical team and the ABC curriculum. 

• Identify infant and family needs, help families access resources, and confirm caregivers can use what is provided. 

Community navigation and family bridging 

• Arrange and provide transport for families to medical, DCS, court, and community appointments. 

• Connect families to local food, housing, clothing, childcare, recovery, and faith community resources. 

• Serve as a bridge between families and BLOOM clinical staff, communicating what is happening in the home and flagging concerns promptly. 

Documentation, safety, and team participation 

• Document each contact accurately

• Report immediately any safety concern, suspected abuse or neglect, or medical change to the CHW Lead or clinical team, and comply with Indiana mandatory reporting obligations. 

• Participate in case reviews with BLOOM leadership and the clinical team, and in ongoing training. 

• Maintain confidentiality and HIPAA compliance and observe clear professional boundaries with families. 

Qualifications

Required 

• High school diploma or equivalent. 

• Residence in or strong ties to the communities BLOOM serves. 

• Lived or work experience caring for families with children. 

• Willingness and ability to complete Indiana Community Health Worker certification and ABC parent coach training within the first year of hire. 

• Valid Indiana driver's license, reliable insured vehicle, and willingness to transport families. 

• Ability to clear DCS background, criminal history, and fingerprint-based checks. 

• Demonstrated ability to work respectfully and without judgment with families affected by substance use, poverty, trauma, and child welfare involvement. 

Preferred 

• Existing Indiana Community Health Worker certification. 

• Prior experience as a doula, home visitor, foster or kinship caregiver, recovery coach, or peer support specialist. 

• Training in trauma-informed care, motivational interviewing, or safe sleep instruction. 

• Bilingual ability. 

Work Environment, Travel & Physical Requirements

• Work is performed in family homes, foster placements, residential recovery facilities, and community settings; home environments vary widely in condition, accessibility, and safety. 

• Regular travel across the service area, with a willingness to transport families in the CHW's own vehicle in urgent situations.

• Must be able to lift and carry supplies of up to 30 pounds, work at floor level with infants, and assist with household tasks such as safe sleep setup.
