Community Health Worker

Community Health Workers provide in-home paraprofessional support to BLOOM-enrolled families and are recruited from the communities BLOOM serves. CHWs also deliver the Attachment and Biobehavioral Catch-up (ABC) Parenting Program and provide practical, in-home support for family thriving by being a critical part of the BLOOM clinical team through hands-on support and connections to community resources.

Full-Time or Part-Time (Flexible Hours) Position
Southern Indiana (Field / Remote Hybrid)
Hybrid (Home-Based Office + In-Home Patient Visits)

Essential Duties & Responsibilities

ABC Parenting Program 

  • Complete parent coach training and certification in the Attachment and Biobehavioral Catch-up model. 

  • Deliver the in-home ABC curriculum to BLOOM families. 

  • Complete session documentation and participate in fidelity review and coaching feedback. 

In-home practical support 

  • Reinforce safe sleep environments and assist with child-proofing the home. 

  • Reinforce caregiving strategies introduced by the clinical team and the ABC curriculum. 

  • Identify infant and family needs, help families access resources, and confirm caregivers benefit. 

Community navigation and family bridging 

  • Arrange transport for families to medical and community appointments. 

  • Connect families to local food, housing, clothing, childcare, recovery, and faith community resources. 

  • Collaborate families and BLOOM clinical staff.

Documentation, safety, and team participation 

  • Document each contact accurately

  • Participate in case reviews with BLOOM leadership and the clinical team, and in ongoing training. 

  • Maintain confidentiality and HIPAA compliance and observe clear professional boundaries with families. 

Qualifications

Required 

  • High school diploma or equivalent. 

  • Residence in or strong ties to the communities BLOOM serves. 

  • Lived or work experience caring for families with children. 

  • Willingness and ability to complete Indiana Community Health Worker certification and ABC parent coach training within the first year of hire. 

  • Valid Indiana driver's license, reliable insured vehicle, and willingness to transport families. 

  • Ability to clear background check, criminal history, and fingerprint-based checks. 

  • Demonstrated ability to work respectfully and without judgment with families affected by substance use, poverty, trauma, and child welfare involvement. 

Preferred 

  • Existing Indiana Community Health Worker certification. 

  • Prior experience as a home visitor, foster or kinship caregiver, recovery coach, or peer support specialist. 

  • Training in trauma-informed care, motivational interviewing, or safe sleep instruction. 

Work Environment, Travel & Physical Requirements

  • Work is performed in family homes, foster placements, residential recovery facilities, and community settings.

  • Regular travel across the service area.

  • Must be able to work at floor level with infants, and assist with household tasks such as safe sleep setup.
