Nurse Practitioner / Physician Assistant 

The Advanced Practice Provider (APP) is the primary clinical provider for infants in BLOOM's in-place primary care model. The APP delivers the full scope of infant well and sick care to infants ages zero to three where those children actually live — private homes, foster and kinship placements, residential recovery settings, and community sites — across the counties of GROW Region 6. Under the NEST initiative, the APP also delivers in-home prenatal education visits in coordination with Valcore Health prior to delivery and participates directly in the launch of a complementary in-place prenatal care model. 

This position is a GROW-enabled expansion: infants in BLOOM's in-place infant primary care program is funded in full by the Rural Health Transformation Program, and this role is central to growing the practice to a standing panel of 22 DCS-involved families. The APP practices under the clinical oversight of the BLOOM CMO and within Indiana scope-of-practice and collaborative practice requirements. 

Essential Duties and Responsibilities

In-place infant primary care 

  • Deliver well-child visits in-place, to vulnerable infants. 

  • Perform and document hands-on developmental screening — ASQ, HINE, M-CHAT and related instruments, with referral to First Steps when indicated. 

  • Educate caregivers on the importance of immunizations on the AAP schedule, partnering with BLOOM RNs to ensure vaccine administration. 

  • Provide acute and episodic sick care in place, including assessment, treatment, prescribing, and escalation to emergency or specialty care when clinically warranted. 

  • Complete patient and caregiver screening covering maternal depression, substance use, intimate partner violence, attachment, and social drivers of health, and make warm referrals to community partners. 

Transitions of care and prenatal work 

  • Receive warm hospital discharge handoffs for vulnerable newborns in coordination with regional delivery hospitals, tertiary centers, and neonatal intensive care units so that infants enter BLOOM care promptly after discharge. 

  • Deliver in-home prenatal education visits in coordination with Valcore Health mothers prior to delivery — introducing the BLOOM model, establishing the care relationship, and preparing families for infant care and the postnatal transition. 

  • Meet with the Valcore Health team as requested, alongside leadership, to support the infrastructure build and launch of in-place prenatal care and to develop BLOOM–Valcore co-management postpartum protocols. 

Documentation, quality, and outreach 

  • Maintain complete and timely clinical documentation in the electronic health record and, once implemented, in the GROW-funded visit planning and outcomes tracking platform; contribute to platform requirements definition and user testing. 

  • Prepare medical and DCS-required clinical documentation and participate in child and family team meetings, case conferences, and reunification planning. 

  • Participate in case review, complex-patient consultation, and documentation review with the CMO.

  • Support presentations, workshops, and direct outreach to the seven regional DCS offices, county health departments, and maternal-infant providers that drive panel growth. 

  • Contribute activity and outcome data quarterly and annually. 

Qualifications

Required 

  • Current, unrestricted Indiana licensure as a Nurse Practitioner (Family or Pediatric) or Physician Assistant, with national board certification. 

  • Minimum of two years of clinical practice experience; pediatric, neonatal, or family practice experience strongly preferred. 

  • Current BLS certification; PALS within 90 days of hire. 

  • Valid Indiana driver's license, reliable insured vehicle, and willingness to travel throughout the region. 

  • Have a working cell phone.

  • Ability to clear DCS background, criminal history, and fingerprint-based checks, and to be credentialed and enrolled with Indiana Medicaid and all three Indiana managed care entities. 

  • Demonstrated ability to deliver clinical care independently in nontraditional, non-clinic settings. 

Preferred 

  • Experience serving DCS-involved families, foster care populations, or child welfare medicine. 

  • Training in trauma-informed care, motivational interviewing, or ACEs-informed practice. 

  • Demonstrated competency with ASQ, HINE, M-CHAT, or Bayley developmental instruments. 

  • Rural health, home-visiting, or mobile integrated health experience. 

Work Environment, Travel, and Physical Requirements

  • Care is delivered in patient homes, foster placements, residential recovery facilities, and community sites; home environments vary widely in condition, accessibility, and safety. 

  • Extensive regional travel across an eleven-county service area; approximately one quarter of visits may be delivered by telehealth. 

  • Must be able to transport and carry clinical equipment and supplies, examine infants at floor level, and lift up to 30 pounds.
