Background
Over the last two decades, adversities and challenges have impacted families, resulting in the birth of infants who are vulnerable and in need of specialized, intensive treatment to mitigate harms.
NEED: A focus on the short- and long-term impacts on the physical, mental and social-emotional health of vulnerable infants.
Opportunity: In-place medical care for vulnerable infants post-discharge, to offer the best chances for the family, the infant, and society.

A proactive model in a traditionally reactive system - in-place care.
BLOOM Model: A collaborative, sustainable, care plan based on evidence-based interventions delivered by uniquely trained medical professionals.
Promoting infant physical, mental, and social-emotional development to mitigate short- and long-term social-emotional challenges.
A financially sustainable, interoperable, replicable model achieving evidence-based endorsement. -
The Results
Our patients exhibiting and experiencing superior physical and mental health outcomes at age two as compared to infants receiving standard care.
We strongly believe that specialized,
in-place care will allow for the best chance to survive and thrive, as well as create positive short and long-term outcomes for these infants and their families.Equitable approaches will be used, along with hope and dignity restored to the most vulnerable in the community.
