The BLOOM Plan.

  • infants, infant medical care, in-place medical care, SUD, substance use disorder, infants of parents with SUD, infants of parents with substance use disorder, babies, baby medical care, restorative infant care, high-risk infant medical care, NAS

    Background

Over the last two decades, adversities and challenges have impacted families, resulting in the birth of infants who are vulnerable and in need of specialized, intensive treatment to mitigate harms.

NEED: A focus on the short- and long-term impacts on the physical, mental and social-emotional health of vulnerable infants.

Opportunity: In-place medical care for vulnerable infants post-discharge, to offer the best chances for the family, the infant, and society.

  • infants, infant medical care, in-place medical care, SUD, substance use disorder, infants of parents with SUD, infants of parents with substance use disorder, babies, baby medical care, restorative infant care, high-risk infant medical care

    The BLOOM Model

A proactive model in a traditionally reactive system - in-place care.

BLOOM Model: A collaborative, sustainable, evidence-based care plan delivered by uniquely skilled healthcare practitioner-partners.

Promoting infant physical, mental, and cognitive development to mitigate short- and long-term social-emotional challenges.

A financially sustainable, interoperable, replicable model achieving evidence-based endorsement.

    The Results

Our patients exhibit and experience superior physical and mental health outcomes at age 2 as compared to infants receiving standard care.

    We strongly believe that specialized,
in-place care will allow for the best chance to survive and thrive, as well as create positive short and long-term outcomes for these infants and their families.

Equitable approaches will be used, along with hope and dignity restored to the most vulnerable in the community.

Enabled infants, empowered parents, and a society that can - BLOOM.
